How it works: Providers engage OxfordPark Health to manage the entire RPCM onboarding process from start to finish. The RPCM Platform enrolls eligible patients and assigns each patient their own personal remote RN Care Coach. In-between face-to-face hospital/practice provider visits, RN Care Coaches are available remotely 24/7 to offer a kind, knowledgable voice on the other end of the line to assist patients with care management, medication/wellness/treatment plan adherence, engaging in prevention education/wellness plans, and monitoring data with remote plug and play devices for early intervention.
